“Bright Health Rewards+” 2022 Program Terms and Conditions

The following terms and conditions apply to the Bright Health Rewards+ Program (“the Program”). The Program is available where law and regulation allow. Please read these Terms and Conditions carefully. The Program’s Terms and Conditions are subject to change at any time. As used in these Program Terms and Conditions, the words “you” or “Rewards Member” mean any member eligible for the Program. The words “we,” “us” and “our” refer to Bright Health Management, Inc., including its subsidiaries, affiliates, agents or administrators servicing the Program (hereinafter referred to collectively as “Bright Health Plan”).

Eligibility:

Rewards Members must:

  • Be an active Bright Health Plan Medicare Advantage Plan member.

Earning Rewards:

All Rewards Members are eligible to earn the following rewards:

  1. Health Risk Assessment ($25)
  2. Annual Wellness Visit ($75)

Check with your doctor to see if you are due for any of the below screenings:

  • Glucose test (HbA1C) = earn $15
  • Retinal eye exam = earn $10
  • Kidney health evaluation = earn $10
  • Mammogram = earn $25
  • Stool test = earn $10
  • Colonoscopy = earn $25

Your eligibility for screening rewards will vary based on your doctor’s recommendations. You can only earn rewards for the screenings you are due for.

  • Each of these rewards can only be earned once in 2022.
  • Rewards will not be fulfilled outside of your plan year; 2022 rewards will be issued until 12/31/2022.
  • Rewards will not be fulfilled for activities completed prior to your plan effective date.

 

The rules for earning rewards are administered by Bright Health Plan. We reserve the right to modify, amend, or terminate rules and/or the ability to earn rewards at any time with or without notice. We are solely responsible for all rewards eligibility standards and determination of the Rewards Card issuance.

Reward Payment:

Once a reward activity is completed, Rewards Members will receive a prepaid reloadable Visa card (the “Rewards Card”) sent to the address on file. This is the Rewards Card for the entire duration of the Program. As Rewards Members complete additional activities that earn rewards, additional funds will be added to the Rewards Card initially received. After completing the first reward activity, Rewards Members can expect their Rewards Card to arrive within 60 - 90 business days. Please keep it in a safe place. We are not responsible for lost or stolen cards. Please do not throw away your card when you have spent the balance.

How to Use your Reward Card:

You may be required to activate your Rewards Card before first use following the instructions on the card.

Rewards dollars can be spent at:

  • Grocery stores, supermarkets
  • Freezer and Locker Meat Provisioners
  • Candy, Nut and Confectionary Stores
  • Dairy Products Stores
  • Bakeries
  • Miscellaneous Food Stores – Convenience Stores, Markets, Specialty Stores, and Vending Machines
  • Eating Places and Restaurants
  • Electronics Sales
  • Household Appliance Stores
  • Home Supply Warehouse Stores
  • Building Materials, Lumber Stores
  • Florist Supplies, Nursery Stock and Flowers
  • Paints, Varnishes, and Supplies
  • Glass, Paint, Wallpaper Stores
  • Hardware Stores
  • Lawn and Garden Supply Stores
  • Service Stations (With or Without Ancillary Services)
  • Fuel Dispenser, Automated

Funds cannot be spent on alcohol, tobacco products, and firearms or to receive cash at checkout or from an ATM.

Reward Expiration:

As long as you are a member of a Medicare Advantage plan with Bright Health Plan, you can access any unused balance on your Rewards Card until 12/31/2022. If you terminate enrollment with Bright Health Plan, your rewards expire immediately.

 

“Bright Health Rewards+” 2021 Program Terms and Conditions

The Bright Health Rewards+ 2021 Program will stop accepting reward submissions and exceptions on 7/31/2022. Unused balance on 2021cards will be swept from cards on 9/31/2022.
